There are a few ways to find Service Plans, but the easiest is by searching for a client, clicking their name, and then clicking on "Plans and Reviews."

Service Plans need to be created for all adult, head-of-household clients with an "active" status in Foothold within 30 days of intake and every six months thereafter.

A service plan is a set of client-directed goals, usually 3-5, that an Avalon Service Coordinator sets with a client.

If you're having issues linking your notes to your plan, set your effective date in the future, mismatched your goal categories, etc., there are a few things you can do yourself to fix these issues:

Remove the signatures from the plan.

To do this:

Follow the same steps you did to sign the plan.

Click "remove e-sign."
IF you have an issue with the plan's dates and/or goals.

a. Remove the signature (see above), if there is one.

b. Go back to "Plans and Reviews."  Click the radio button next to the plan you just removed the signatures from.  Click "edit."

c. Bring your mouse cursor over the effective date.  A blue "x" should appear.  Click it to remove the effective date.  Then, click "save."
d. Repeat this process for the "done date."

e. make any fixes to the service plan that you need.

f. Return to "attributes." Set the effective date (and any other dates) to the SAME date. This date should be the date on which the plan STARTS.

g. Sign.

⭐If you've set up your service plan correctly, you'll see a gray box labeled "Service Types on ISP Today."  If not, either the plan is not locked with an effective date, the effective date is in the future, or a goal's service types (ex: housing stability, financial, etc.) don't match the service type for the same goal's corresponding Case Manager task.
⭐Make sure the "service type" for the progress note (ex: housing stability") matches the "service type" for the goal you're trying to link the note to!

After you have set up your Service Plan, you're ready to write Plan-linked progress notes to demonstrate how your services relate to the client-directed service plan.

Position and resize the waypoint to include the content you want to present.
